Information to send from your office
Many patients referred to our offices have already had a workup performed to some level. Please ask your patients to bring pertinent imaging studies like xrays, MRIs, CT scans, bone scans and ultrasounds to their visit. Also, copies of medical records, previous operative reports, office notes, medications and insurance information will be extremely helpful, avoid office delays, and speed treatment.
